Pediatric Dosage Exception Identified Before Dispensing

Background

A 4-year-old child with a respiratory condition was prescribed montelukast 5 mg chewable tablets once daily. For children younger than six, the FDA-recommended dose is 4 mg daily. The 5 mg dose is approved for children ages six and older. The prescribed dose exceeded age-based recommendations and could increase the risk of adverse effects.

What we did

HyperCare flagged the dosing error and PharmPix’s clinical team contacted the prescriber to validate. Because it exceeded FDA pediatric recommendations, the medication was not approved as written. The team provided an age-appropriate dosing recommendation.

The impact

  • Prevented inappropriate pediatric dosing
  • Reduced the risk of dose-related neuropsychiatric adverse effects
  • Reinforced age-appropriate prescribing practices
  • Protected patient safety before the medication was dispensed

Clinical protection happens at the point decisions are made. PharmPix helps identify potential medication risks and brings clinical protection to the moments that matter.
